Kelly Smith serves as LifeStream’s Dementia Outreach Specialist covering area 9 (Fayette, Franklin, Rush, Union, and Wayne counties). Kelly earned a degree in Human Services from Purdue University and has worked in Human Services fields for the last 15 years. Since joining LifeStream, Kelly has obtained her Certified Dementia Practitioner credentials (CDP) from the National Council of Certified Dementia Practitioners and has worked on implementing Dementia Partnerships, Education, Resources, and Memory Screenings in our communities. Kelly works with community partners and Health Networks to provide evidence-based trainings and development, community education, and partnerships to help improve the quality of life, care, and services for persons living with dementia.

Chelsey Krofta is the Caregiver Programs Coordinator at Lifestream Inc. She covers Area 9 which includes Wayne, Franklin, Fayette, Union and Rush Counties.

Chelsey has her bachelors in Communication Studies with a background in marketing, promotions, employee wellness, fundraising, volunteer and program management. She is connected to this work because she also has loved ones living with dementia.

Chelsey is proud to be offering the power of knowledge to individuals and breaking down the stigma.
